However, the volume of people with access to a reablement service should and will continue to rise. Alongside this continuing growth in demand and provision, there is enormous scope to improve performance levels in terms of maximising levels of potential independence for service users. This conference examines and discusses what good practice looks like, for the service user and for service providers. Through shared insights and expertise delegates will identify how they and their organisations can embed this in everyday practice. Speakers include: • Gerald Pilkington, Health and Social Care Consultant, Gerald Pilkington Associates and former CSED Lead, Homecare Re-ablement, Department of Health • Professor John Bolton, Consultant, JRFB and Director of Adult Social Care in Westminster • Steve Tingle, Director of Adult Commissioning and Personalisation, Adult Services, Blackburn with Darwen Borough Council • David Gray, Group Manager, Home Care, Birmingham City Council Who should attend? • Local authorities – reablement managers, care assessment managers, reablement team managers, reablement support managers, occupational therapists, risk managers, safeguarding adults teams, social workers, early intervention managers • NHS - primary care ward managers and community based workers, GPs, patient discharge teams, clinical commissioning groups, physiotherapists • Age Concern local branches • Health & Wellbeing Boards • Intermediate care providers • Older people’s self-advocacy groups • Social housing (sheltered housing trusts/charities/associations) • Assistive technology providers • Home carers • Voluntary and community support organisations
