Our Reablement services support adults, who may have had a change to their health, routine or lifestyle and are finding it difficult to live independently. We support frail and vulnerable adults in their own homes following a stay in hospital or alternative to prevent them from being admitted to hospital. Our short-term care team helps people to remain living at home safely and independently. The support we offer can be in the form of helping someone with their personal care or medication, to the provision of a piece of equipment to promote a person’s independence.

Our caring team make our customer’s daily lives easier and provides assistance with gaining access to both specialist equipment and longer-term support if required. Within this role, you will provide practical, personal and emotional care to customers, whilst determining on-going reablement requirements and establishing appropriate support plans.

You will be highly organised and a good communicator; with the time, patience and passion to provide the highest quality support. Our support workers will always be happy to go the extra mile to accommodate and support our customer’s needs and desires.
