Aspen Hamilton Care Management

Care staff

Providing care and support to people within their own home.

As a support member of the care team, you will carry out all the appropriate duties necessary to fulfil each individual client’s needs as stated in their Care Plan and under the direction and supervision of the Care Manager.

Aims and Objectives:

To meet the individual needs of each client according to the care plan

Carry out assigned tasks associated with the care of the client to the required standard

Maintain effective communications with the client, the Care Manager and where appropriate other health care professionals.

Report all matters relating to the client’s care especially any changes, to the Care Manager

Be conversant with and comply with the Code of Practice of Aspen Hamilton Care Management and relevant Policies and Procedures

Attend training and reviews as required by the Care Manager



  • Previous experience in adult personal and/or social care
  • Good communication skills
  • Good literacy and numerical skills
  • Able to work well both as a member of a team and alone with clients
  • Understanding of equal opportunities diversity and personal choice
  • SVQ2 in Health and Social Care (desirable)
