Job purpose:

To undertake care work within the community in a way which will preserve the dignity, privacy, choice, independence, fulfilment and rights of the service user and his/her usual carer/family members/advocate

 

Key responsibilities and accountabilities:

  • Attends to care and support tasks as agreed and in line with the service users care plan
  • To maintain good communication and develop effective working relationships with service users.
  • To liaise with agency staff and/or other professionals as necessary or as directed
  • To develop good communication with any carer or family member in the service users home
  • To develop skills and understanding of the special needs of service users who may be physically or mentally frail, or suffer from any other disability
  • Provide personal care of an intimate nature, such as washing, dressing, toileting (where required)
  • Supporting service users around with their mobility and independence, helping users in and out of bed,bath,wheelchair and chairs following agreed risk assessments
  • Where assessed undertake the monitoring, prompting and overseeing of service users medication
  • Assist service users to preparing and/or heating meals drinks and snacks as necessary.
  • Undertake domestic duties such as cleaning ,laundry and other general household duties
  • Assist service users with their groceries and other shopping tasks. This may include escorting service users to the shops
  • To alert supervisors/managers of any changing circumstances of service users conditions
  • Ensure a safe as possible living environment for service users whilst respecting service users choice and rights
  • Take responsibility for the safe handling of property and equipment belonging to the service user
  • Work as part of a team and equally be able to work alone
  • Dress appropriately for post (protective clothing and uniforms are provided by Gold Hill Homecare)
  • To be prepared to work weekends and Bank Holidays as agreed
  • To be accountable for your work practice and take responsibility for maintaining and improving your knowledge and skills
  • To fully co-operate in mandatory training, supervision, appraisals and staff meetings as required
  • Be willing to travel between service users own homes
  • Be available and willing to undertake any other tasks specified by line management relevant to the role and to the needs of the service users.
  • Record all visits to service users home in their care plan
  • Maintain work records e.g. care record sheets, medication forms, financial transaction sheets, receipts
  • To carry all care calls at times stipulated on your work rota
  • To notify without delay with regards to any changes made to your programme of work
  • To adhere to all Gold Hill Homecare policies and procedures
