Job Summary
We are seeking an experienced, proactive, and values-driven Care Coordinator to join our dedicated team. This hybrid role combines 10 hours of care coordination each week with hands-on care work to complete your contracted hours. It’s ideal for someone who wants to grow professionally while staying closely connected to frontline care.
Role Responsibilities

Completing 10 hours per week of care coordination tasks, including scheduling, communication, and supporting the day-to-day running of the service.

Delivering direct care for the remainder of your hours to maintain a strong understanding of service needs.

Working two evenings per week (4pm–10pm) to gain full oversight of evening operations.

Covering some weekend shifts (morning, lunch, tea, and bed calls) as required for the role.

Providing emergency cover for care calls when needed.

Building effective working relationships with GPs, pharmacists, OTs, PTs, social workers, service users, and families.

Managing low-level complaints with professionalism and empathy.

Maintaining excellent timekeeping and accurate record-keeping.

Ensuring all practice aligns with Health & Social Care regulations, including CQC requirements.

Reporting weekly to the Registered Manager on service performance, concerns, and priorities.

Actively contributing to an open, transparent, and supportive team culture, where communication is honest and everyone feels valued.
