I can confirm a full assessment was carried out with you and your wife documenting your preferences relating to the care and support you required following your 6 weeks of reablement which is funded until a full assessment is carried out. We would like to apologise for any errors in communication that may have occurred following the referral process from the Hospital Discharge team. We proudly work alongside them to make these transitions as smooth as possible by both organisations. We appreciate that it can be a difficult time with a large amount of information given in a short period. Both teams do try to ensure that communication is clear at all stages and are sorry for any confusion or shortfall on this occasion.
