Carers reminded her during morning visits, and a welfare call was made later that day to check she was comfortable - which she confirmed. We accept this may not have been clearly shared with the wider family. On the final day, we were instructed by CCG to conclude care after the morning visit. That evening, when we were informed the new provider hadn't attended, we offered to return for the bedtime visit - this was declined. We're cooperating fully with social services and welcome their review. If any improvements are identified, we'll take them on board and act on them.
