As you are aware, whilst providing care, we had a meeting with social services in regards to your attitude and behaviour towards our staff. Following the incident you refer to, we followed correct policy and procedures by reporting to safeguarding, social services and via our formal complaint process. The findings of the investigation were sent to you within 14 days of receipt. We tried our utmost to work alongside you, however, due to the irreparable breakdown in the relationship between yourself, care staff and HQ we felt that for the safety of both parties it was in the best interest of all involved to refer your care package back to social services, as was discussed verbally with you at that point.
