Developed by West Hampshire Clinical Commissioning Group, RESTORE2 is a physical deterioration and escalation tool for care settings including people in their own homes and based on nationally recognised methodologies including early recognition (Soft Signs), the national early warning score (NEWS2) and structured communications (SBARD).
Its aim is to detect signs of deteriration early enough so that prompt treatment can be sought to respond to a potential medical event, and to access appropriate health services from your GP, Pharmacist, District Nurse, Paramedic etc thereby avoiding unecessary hospital admission.
