Job summary

Main Responsibility:

To be part of a team in providing support for Service Users with enduring mental health problems and, or, learning difficulties within the Community in line with the individual’s person centred plan.

Specific Tasks:

  • To assist with the planning and co-ordination of the delivery and support services to Service Users ensuring that personal, physical, emotional, social and spiritual needs are met.
  • To ensure that the service provides opportunities for Service Users to exercise determination and choice and to participate in the life of a wider community.
  • To comply with the General Social Care Council Code of Conduct for Social Care Workers.  To ensure that the personal dignity of all Service Users is upheld and respected.
  • To contribute to the development of, and assist, with the implementation of individually planned programmes of support and activities according to each Service Users needs.
  • To develop a good working relationship with the Service User and, when appropriate, use their families, friends and advocates.
  • Undertake direct tasks and participate in shift patterns as required.
  • Undertake any reasonable tasks related to the provision of support as required by the Line Manager.

·  To report any problems/changes/complaints concerning the service or Service User to the Line Manager.

·  As and when designated by your Line Manager, complete the required paperwork for the service in their absence.

Helping Service Users with daily living skills including:-

  • Assisting with budgeting monies and help with paying utility bills.
  • In some cases, assist with personal hygiene and washing etc. Assistance is also needed with shopping, cleaning of their home, preparation of meals and laundry.
  • Administration of prescribed medication.
  • Remaining non-judgmental and reporting concerns to your Line Manager.
  • Making daily entries in the appropriate documentation.
  • Attending training and supervision sessions and accepting guidance from senior staff.

General

To uphold all policies and procedures as defined in the Human Resources and 

  • Quality Management systems.
  • To treat all people with dignity and respect, always promoting an environment free from bullying, harassment and intimidation.
  • To participate in meetings or as otherwise directed.
  • To act as a nominated key holder, if required.
  • Any other duties as deemed appropriate by the Line Manager.

Personal Development

  • To actively participate in own supervision and personal review processes in accordance with company policy.
  • To undertake any training that is deemed needed and mandatory by the company.

  • Report immediately to your line manager or designated person any defects in equipment or the working environment.
