Job summary

We are a reputable and compassionate domiciliary care provider, focused on supporting individuals to live independently in their own homes with dignity and respect. We are currently looking for a qualified Nurse (RGN/RMN) to join our team, bringing clinical expertise to the assessment and care planning process, and supporting our care staff in delivering outstanding care.

Role Overview:
This is an exciting opportunity for a registered nurse who enjoys autonomy and variety in their work. You will take a lead role in conducting initial assessments, care reviews, risk assessments, and supporting with complex care planning. You’ll work closely with the Care Manager and help oversee the quality and safety of our services, ensuring person-centred care for every client.

Key Responsibilities:

• Conduct comprehensive initial assessments for new service users.

• Complete regular care plan reviews and risk assessments.

• Provide clinical oversight for complex care packages.

• Liaise with families, GPs, district nurses, and other healthcare professionals.

• Support and advise care staff on clinical and care-related matters.

• Contribute to staff training and development from a clinical perspective.

• Assist with audits, compliance, and quality improvement initiatives.

Requirements:

• Registered Nurse (RGN or RMN) with valid NMC pin.

• Experience in community, home care, or clinical assessments.

• Strong understanding of CQC requirements and person-centred care.

• Excellent communication and interpersonal skills.

• Ability to work independently and manage your own caseload.

• A full UK driving licence and access to a vehicle.
