*Job Summary:*
As a Care Manager, you'll be responsible for overseeing the delivery of homecare and supported living services to our clients, ensuring their needs are met, and their well-being is prioritized. You'll lead a team of care professionals, provide guidance and support, and work closely with healthcare professionals to ensure seamless care.

*Key Responsibilities:*

- Manage a team of care professionals, providing guidance, support, and development opportunities
- Assess client needs, develop care plans, and ensure their implementation
- Monitor and evaluate care services, identifying areas for improvement
- Collaborate with healthcare professionals, family members, and other stakeholders to ensure comprehensive care
- Ensure compliance with regulatory requirements and company policies
- Develop and maintain relationships with clients, families, and healthcare professionals
- Manage scheduling, rotas, and staffing to ensure efficient delivery of care services
 

Skills / Qualifications:

  • - Registered nurse or social worker qualification (or equivalent)
  • - Proven experience in care management or a similar role
  • preferably in homecare or supported living service
  • - Strong leadership and management skills
  • - Excellent communication and interpersonal skills
  • - Experience with care management software and technology
  • - Ability to work independently and as part of a team
  • - Strong problem-solving and analytical skills
  • - Experience with care management software and technology.
