Job summary
We are currently seeking a dedicated and experienced Care Coordinator/Field Supervisor to join our team in Harrow and Ealing. In this vital role, you will be responsible for supporting and monitoring the performance of domiciliary care workers, ensuring the smooth setup of new care packages, and maintaining high standards of client satisfaction.
You will play a key part in creating and supervising optimised staff schedules that meet both client needs and workforce capacity. Working closely with the Registered Manager, you will also conduct field supervisions, competency assessments, and contribute to the ongoing professional development of care staff.
DUTIES / KEY RESPONSIBILITIES
Care Coordination & Support Delivery Plan and maintain staff rotas in line with service user needs and compliance requirements,
Conduct new service user assessments, create transition plans, and support care plan reviews
Support new packages by setting up systems, scheduling introductions, and arranging staffing.
Respond quickly to rota gaps and emergencies ensuring continuity of care.
Carry out competency checks and spot checks to ensure compliance with care standards.
Help assess and supervise staff, with spot checks, supervisions, observations, client visits etc. And help maintain their training records to ensure that they are compliant
Overseeing Quality Assurance/Client and Staff Feedback
Liaise with the Registered Manager to ensure the right number of Care Assistants, with the right skills mixes, are recruited to meet the needs of the business. Arrange cover for Care Assistant sickness, absenteeism or holidays.
Liaise with families, professionals, and external stakeholders to ensure collaborative care delivery
Handle general office admin tasks such as filing. Letter writing, and responding to calls/emails
Liaising with Staff and Clients for Contracts and Services.
Resolving queries and addressing issues with clients /Families/Staff/Social Services
Resource allocation of Staff to Clients for shifts/services
Maintain balanced workload of Care Staff

Schedule: Monday to Friday
Hours: Full-Time.
Type: Permanent but with Initial 3 months of probabtion period.

Skills / Qualifications:
- Minimum 2 years of experience as Care Coordinator Full UK driving licence Minimum NVQ Level 2 and willing to do Level 3
