Are you a compassionate and experienced leader looking to make a difference in the lives of others?

We are seeking a dedicated a Support Worker to join our expanding team and oversee the delivery of high-quality, person-centered support to individuals with Learning disabilities or other support needs, enabling them to live as independently as possible across our three Supported Living homes in Exmouth.

Key Responsibilities:

Person-Centred Support:

  • Develop and maintain strong relationships with individuals, building trust and understanding their unique needs and aspirations.
  • Collaborate with individuals to create personalised support plans that reflect their goals and preferences.
  • Promote choice, independence, and dignity in all aspects of daily life.
  • Daily Living Skills:
  • Assist with activities of daily living as needed, including personal care, meal preparation, and household chores.
  • Teach and encourage individuals to develop essential life skills, such as budgeting, managing medication, and accessing community resources.
  • Support individuals in maintaining a clean and safe living environment.
  • Community Integration:
  • Facilitate opportunities for individuals to participate in social activities, recreational pursuits, and community events.
  • Assist with transportation and accompany individuals on outings as required.
  • Encourage and support individuals to build and maintain relationships within their community.
  • Health and Wellbeing:
  • Monitor individuals' health and well-being, ensuring timely access to medical appointments and treatments.
  • Administer medication as prescribed and document medication administration accurately.
  • Promote healthy lifestyle choices, including regular exercise and nutritious eating.
  • Record Keeping and Reporting:
  • Maintain accurate and up-to-date records of support provided, including incident reports, medication administration logs, and progress notes.
  • Communicate effectively with colleagues, supervisors, and other professionals involved in the individual's care.
  • Participate in regular supervision and team meetings.

Skills / Qualifications:

  • Essential Skills and Experience: Relevant experience in supporting individuals with disabilities or other support needs. Strong interpersonal and communication skills. Ability to work independently and as part of a team. Patience
