Are you a compassionate and experienced leader looking to make a difference in the lives of others?
We are seeking a dedicated a Support Worker to join our expanding team and oversee the delivery of high-quality, person-centered support to individuals with Learning disabilities or other support needs, enabling them to live as independently as possible across our three Supported Living homes in Exmouth.
Key Responsibilities:
Person-Centred Support:
- Develop and maintain strong relationships with individuals, building trust and understanding their unique needs and aspirations.
- Collaborate with individuals to create personalised support plans that reflect their goals and preferences.
- Promote choice, independence, and dignity in all aspects of daily life.
- Daily Living Skills:
- Assist with activities of daily living as needed, including personal care, meal preparation, and household chores.
- Teach and encourage individuals to develop essential life skills, such as budgeting, managing medication, and accessing community resources.
- Support individuals in maintaining a clean and safe living environment.
- Community Integration:
- Facilitate opportunities for individuals to participate in social activities, recreational pursuits, and community events.
- Assist with transportation and accompany individuals on outings as required.
- Encourage and support individuals to build and maintain relationships within their community.
- Health and Wellbeing:
- Monitor individuals' health and well-being, ensuring timely access to medical appointments and treatments.
- Administer medication as prescribed and document medication administration accurately.
- Promote healthy lifestyle choices, including regular exercise and nutritious eating.
- Record Keeping and Reporting:
- Maintain accurate and up-to-date records of support provided, including incident reports, medication administration logs, and progress notes.
- Communicate effectively with colleagues, supervisors, and other professionals involved in the individual's care.
- Participate in regular supervision and team meetings.
Skills / Qualifications:
- Essential Skills and Experience: Relevant experience in supporting individuals with disabilities or other support needs. Strong interpersonal and communication skills. Ability to work independently and as part of a team. Patience
