Role Overview:

We are seeking a dedicated Care Coordinator to join and support the growing team at our Basildon & Brentwood branch. The role involves coordinating the care support workers (support workers/healthcare assistants) and ensuring that care packages across Basildon and Brentwood are delivered safely, consistently, and in line with HOPE’s values and CQC standards.

The post holder will work closely with the Managing Director, Team Leaders, and senior staff to support the recruitment, induction, rota planning, and monitoring of our care staff across the region.

Key Responsibilities:

  • Staff Coordination: Plan rotas and schedule care visits for support workers/healthcare assistants, ensuring timely delivery of care across Basildon and Brentwood.
  • Recruitment Support: Assist in recruiting and onboarding 12 support workers, 2 team leaders, and other care staff, in collaboration with the recruitment lead (Michael) and HOPE’s management team.
  • Staff Induction: Organise and support induction programmes for new staff joining the Basildon and Brentwood branch.
  • Care Planning: Draft and update care plans and risk assessments in line with CQC standards and client needs.
  • Compliance: Ensure services are compliant with local authority requirements, CQC regulations, and HOPE’s internal policies.
  • Liaison: Communicate regularly with local authority representatives, families, healthcare professionals, and the care team to maintain service quality.
  • Safeguarding: Promote safeguarding principles and ensure any concerns are reported appropriately.
  • Logistics Support: Assist in planning transport logistics for care workers who cover jobs outside Essex, especially in Basildon and Brentwood.
  • Documentation: Maintain up-to-date service user records, visit logs, and other relevant documentation via HOPE’s online systems.
  • Team Support: Support team leaders in addressing operational challenges, staff development, and maintaining morale.
  • On-Call Duty: Participate in the on-call rota as required.

Requirements:

  • Minimum 1 year experience as a Care Coordinator or in a similar role in the UK care sector.
  • Good understanding of domiciliary care and supported living for children, young people, and adults.
  • Knowledge of CQC regulations and local authority requirements.
  • Strong IT, organisational, and communication skills.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Full UK driving licence and access to a car.
  • NVQ Level 3 in Health and Social Care (or working towards).
