As the Care Coordinator, you will be responsible for supporting in the day-to-day running of the care company and the people who we support. Ensuring that high levels of emotional and physical care are provided together with the care package for each person. Working together with the team and ensuring all staff are all fully supported.

Key Responsibilities

  • Scheduling and coordinating care visits in line with individual care plans
  • Maintain effective working relationships with the care team
  • Managing care staff rotas and ensuring continuity of care
  • Conducting risk assessments and care reviews
  • Liaising with clients, families, and healthcare professionals
  • Supporting recruitment, induction, and training of care staff
  • Participating in the on-call rota to provide out-of-hours support
  • Carry out audits to monitor the service performance

What We’re Looking For:

  • Previous experience in a care coordination or scheduling role (domiciliary care preferred)
  • Excellent communication and organisational skills
  • Confident with IT systems and care management software
  • Strong leadership qualities and the ability to work under pressure
  • A genuine passion for delivering person-centred care
  • A full UK driving licence and access to a car
  • Passion for helping people.
  • Level 5 Diploma in Leadership and Management for Health and Social Care is desirable but not necessary.
  • A strong understanding of local and national regulations governing home care agencies.
