 About You and the Role
Reporting into the Head of Integrated Care, this role will focus on
  the smooth running of care delivered within Auriens Chelsea,
  monitoring the standards of care delivered, focusing on
  resident-centred services and the overall wellbeing of residents.
  
 This role will be in liaison with the Draycott clinical team to
  ensure standards of care practice are upheld. They will establish
  close working relationships with residents, the Draycott care team and
  the wider Auriens operations team. They will be a strong communicator
  and will know what is going on in the building, will work
  collaboratively with those teams and support to find efficient
  solutions to issues as they arise. The Care & Support Coordinator
  will be a Draycott Ambassador as well as an Auriens Ambassador.
  
 This is a full-time, onsite role based Monday to Friday (5 days
  per week).
  
 Key Responsibilities
General
- Be a Draycott and Auriens Ambassador
- Be the champion for a holistic wellbeing approach at Auriens across all teams, coordinating an exceptional and seamless care and support service delivery
- To ensure confidentiality and professional boundaries are always maintained
- Attend all operational meetings that require a care and support
    operational presence
 
 Team
- Regular team briefings (to include Draycott team) to keep each member of the team informed of incidents and updates from day to night shift
- Daily oversight of the Day and Night Care Team working with residents at Auriens Chelsea, ensuring that full weekly rotas are covered, at least one month in advance for planned activity. Touchpoints with the night team may require alternative shift times.
- Team leadership to ensure colleagues in the Care Team are clear about their duties and responsibilities
- Ensure that payroll for all Auriens Care Team is processed accurately on a monthly basis
- Escalate training needs for both the Care team and wider Operations team to the Head of Integrated Care
- Delivery of highly effective communication across the business to key stakeholders (Auriens and Draycott as necessary), ensuring that all relevant parties know what is going on with regards to Care delivered in Auriens Chelsea
- Collaborate with the relevant team members to develop health and wellbeing programmes
- Partner with Kyros, our onsite Health and Wellbeing Experts to ensure a full holistic wellbeing approach is taken
Residents
- Maintain good relationships with Auriens Residents to ensure the right level of support is being delivered and maintained
- Responsible for the safe onboarding and ongoing need of our clients with care needs
- Keeping in contact with our residents and their families on a regular basis
- Build and maintain relationships with all residents and understand their individual health and wellbeing needs
- Play a wellbeing role in planning and/or participating in resident trips and outings
- Build relationships with resident’s families and advisors
- Encourage and empower Residents to achieve personal goals as outlined in their Individual Needs Assessment Plan, and identify activities and events that may contribute to individual and collective wellbeing in line with the Auriens Wellbeing Model.
- Provide wellbeing, care and support advice and guidance to residents and other colleagues, escalating to the Head of Integrated Care and Auriens Care Team to ensure the right resource is deployed
Care & Support
- Maintain case notes and records of all Resident interactions.
- In liaison with the Draycott Clinical team, document changes in resident care needs, to ensure individual care plans and risk assessments are up to date
- Escalate residents that require immediate attention to the appropriate service
- Liaise with multidisciplinary healthcare professionals, such as Doctors, Physiotherapists, Occupational Therapists and all members of the multidisciplinary team.
- To ensure that our residents have the best possible care, at the right time, in the right place to ensure optimum health & wellbeing outcomes. All events, actions and outcomes must be documented.
- To work with the Head of Integrated Care to ensure that all standards for compliance are adhered to for the delivery of excellent care standards at Auriens Chelsea
- Ensure effective utilisation of the Auriens Care Team for care and support bookings.
- To book in and personally provide cover for ad hoc care visits for Auriens residents in case of need where the Care Team are already engaged with other planned activity, including escorting to and from appointments, emergency / urgent care, and assistance with medication administration / short term personal care, while transferring ongoing support needs to the Care Team.
- To ensure that chargeable services are billed to clients in accordance with Draycott’s standard procedures
- Coordinate with third parties to access expertise and resources as needed
- Liaise with emergency services or other healthcare providers

Essential requirements
- An enhanced disclosure with the Disclosure and Baring Service
- Satisfactory References
- Occupational Health Clearance
- Solid experience as a Care and/or Support Worker in a domiciliary setting
- Solid experience as a Care Coordinator in a domiciliary setting
Experience and attributes
- Health and Social Care qualifications, to level 3 or above
- 3 years’ experience in a professional care setting
- Experience working with remote field-based teams would be helpful
- Professional care experience ideally in a domiciliary care/ Residential and IRC setting
- Effective communication skills and empathy toward the varying issues faced by our clients
- Have excellent customer service skills that are compatible with a luxury service delivery
- Highly organised and detail oriented
- Computer literate
- Ability to maintain high standards with excellent listening skills
- Ability to support Business Development and appropriately promote our services
- Excellent communication and interpersonal skill
