Job Summary:

PROMPT HEALTHCARE STAFFING LTD is seeking an experienced and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring that our clients receive high-quality care and support. You will be responsible for coordinating care services, communicating with clients, families, and healthcare professionals, and ensuring that care plans are implemented effectively.

Responsibilities:

* Coordinate care services for clients, including assessments, care planning, and review

* Communicate effectively with clients, families, and healthcare professionals to ensure seamless care delivery

* Participate in the induction and shadowing of Care Workers

* Develop and implement care plans that meet the needs of clients

* Monitor and evaluate the quality of care services provided

* Collaborate with multidisciplinary teams to ensure comprehensive care

* Maintain accurate and up-to-date records of client care

* Provide emotional support and empathy to clients and their families

Requirements:

* 1 year of experience in a care coordination or similar role

* Knowledge of scheduling home carers to ensure all clients receive a good quality visit.

*Maintain clear and accurate written records and follow statutory reporting procedures.

* Strong communication and interpersonal skills

* Ability to work effectively in a team environment

* Excellent organizational and time management skills

* Ability to prioritize tasks and manage multiple responsibilities

* Knowledge of healthcare policies and procedures

* Ability to maintain confidentiality and handle sensitive information
