Job Summary

About the Role

We are looking for a Care Coordinator who genuinely loves what they do. This isn’t just a scheduling job — it’s a role for someone with real passion for care quality, who wants to take ownership, raise standards and grow into a Registered Manager as our service expands into complex care and supported living.

You’ll sit at the heart of our operations: coordinating care, developing care plans, onboarding new service users, managing key stakeholder relationships and supporting our frontline team. You’ll have direct access to the management team and real scope to make a difference.

What You’ll Be Doing

Coordination & Operations / Care Planning

  • Roster and coordinate care visits for timely, consistent delivery
  • Provide on-call support on a rota basis
  • Maintain accurate records compliant with CQC, GDPR and DPA 2018
  • Contribute to bids and tenders for new care packages
  • Lead onboarding of new service users — building trust and coordinating person-centred starts
  • Develop, implement, and review personalised care plans including complex needs, dementia and end-of-life care
  • Provide hands-on care support including personal care and medication management

Stakeholder Relationships & People Quality

  • Manage relationships with district nurses, health case managers, social workers, local authority commissioners, service users, and families
  • Supervise frontline staff through spot checks, observations and care audits
  • Liaise with NHS commissioners, social workers and healthcare professionals
  • Deliver on-the-job training and mentoring to care staff
  • Drive quality assurance and support CQC inspection readiness
  • Embed a culture of high standards and genuine compassion

What We’re Looking For

Skills and experience matter — but so does character. We want someone with a genuine passion for care, who holds themselves to the highest standards and treats every service user with dignity and respect.

Essential

  • Minimum 3 years’ experience as a Care Supervisor or Coordinator
  • Any degree or NVQ Level 3+ qualification
  • Passion for care and commitment to quality
  • Proficient in Microsoft 365; comfortable learning new systems
  • Valid UK driving licence and access to a vehicle
  • Strong communication skills — warm, confident, trusted by families and professionals alike
  • Well-organised and calm under pressure
  • Flexible across a 7-day rota including evenings, weekends and bank holidays
  • Willing to complete an Enhanced DBS check

Desirable

  • Level 5 in Health & Social Care or Leadership & Management (or working towards — we’ll support you)
  • Experience with CQC inspections and quality assurance
  • Familiarity with complex care or supported living
  • Experience with digital care platforms (e.g. Nourish, CarePlanner, Person Centred Software, Log my Care)
  • Working towards or interested in Registered Manager registration
