Job Summary
About the Role
Classic Resources is looking for an experienced and highly organised Care Coordinator to join our growing domiciliary care team.
This is a rewarding, fast-paced role where you will help ensure our clients receive safe, reliable and person-centred care in their own homes.
You will coordinate care packages, manage staff rotas, support care workers, communicate with clients and families, and work closely with healthcare professionals and other agencies.

Key Responsibilities
- Coordinate the day-to-day delivery of domiciliary care packages; create and manage staff rotas, ensuring all care visits are covered.
- Match care workers to clients based on needs, skills, availability and continuity; arrange emergency cover for sickness and staff absence.
- Maintain regular communication with clients, families and care workers; monitor changes in clients' needs and arrange appropriate action or reviews; ensure care plans, risk assessments and records are accurate and up to date.
- Support and guide care workers with day-to-day queries and care delivery; monitor missed, late or cancelled visits and take appropriate action; liaise with GPs, district nurses, social workers, NHS teams, local authorities and other professionals.
- Support safeguarding, incident reporting, complaints and medication-related processes; assist with CQC compliance, audits, quality monitoring and service improvement; promote dignity, independence, choice and high-quality person-centred care.
- Participate in an on-call rota where required.
- Full UK driving licence and access to a vehicle, where required.
Desirable
- Level 3 Diploma in Adult Care or equivalent.
- Previous experience coordinating domiciliary care packages.
- Knowledge of CQC requirements.
- Experience with electronic care planning and rostering systems.
