Job Summary
Almond Flex Care is seeking an experienced and compassionate Care Coordinator to support the effective planning, coordination and delivery of high-quality care services for children, young people and/or adults.

The successful candidate will coordinate care packages, allocate suitable care staff, communicate with service users and families, and work closely with social workers, local authorities, healthcare professionals and other stakeholders to ensure safe, person-centred and responsive care.

Key Responsibilities

  • Coordinate and monitor care packages and staff rotas.

  • Match suitably trained and experienced care workers to service users.

  • Maintain accurate care records, care plans and risk assessments.

  • Liaise with families, social workers, local authorities, NHS professionals and other agencies.

  • Monitor the quality and continuity of care and respond promptly to concerns.

  • Support safeguarding, incident reporting and CQC compliance.

  • Conduct care reviews and identify changes in service-user needs.

  • Support recruitment, induction, supervision and ongoing training of care staff.

  • Respond to service-user and staff queries and help resolve issues professionally.

  • Ensure services are delivered in accordance with Almond Flex Care policies, care plans and regulatory requirements.

Essential Requirements

  • Level 3 qualification in Health & Social Care or equivalent.

  • At least 5 years’ relevant experience working in either adult social care, children’s services, or both.

  • Good understanding of safeguarding, person-centred care, risk management and care planning.

  • Strong communication, organisation and problem-solving skills.

  • Ability to work effectively with families, professionals and care staff.

  • Good knowledge of CQC requirements and social care standards.

  • Ability to work independently and as part of a multidisciplinary team.

  • A full UK driving licence and access to a vehicle would be desirable.

Desirable

  • Experience coordinating domiciliary care, supported living, day services or complex care packages.

  • Experience working with local authorities, NHS discharge teams and social workers.

  • Experience using electronic care management systems such as Nourish.

  • Knowledge of medication, moving and handling, safeguarding, autism, learning disabilities and positive behaviour support.
