Thank you for the review and for the opportunity to evidence we have learned lessons. Understandably your father wasn't himself following the temporary care being provided to him. While you were on holiday his routine was changed and he became very unsettled and agitated - indicated by the challenging behaviours staff had to manage, such as lashing out, hitting, biting and spitting. I am sorry to learn your father contracted e-coli, and I can confirm, the two staff in question, who allowed food to cool to room temperature and then allowed him to eat one bite of this before removing it from him, were removed from home care provisions. All Home Care staff, have been retrained in hydration and nutrition, and subsequent competency assessments and observational supervisions have evidence that lessons have been learned. Both Leeds and Bradford Local Authorities Safeguarding teams have confirmed that lessons have been learned, and that no future risk will be posed to other customers.

We aim to provide excellent quality home care to keep customers safe and comfortable in their own home. We believe the customer comes first and this means we provide the care and support needed in the way they want it. Services are regularly reviewed to meet the needs of the customer, and in this circumstance due to the refusal of care and support we were unable to help for as long as we would have liked. I only hope our services may be tailored to be of use in the future, when his condition has become more manageable for him and all concerned.
