Staff had not read patient notes in most cases. They would change times and not notify anybody and when questioned changes happened late at night so couldn't message or text to let client know. Some staff didn't know basic body parts eg knees for ankles. Damaged property with no apology. Notes of visit with different name of client. Reluctance to carry out tasks listed in notes. Tasks lised as being done when not carried out. Lack of basic hygiene, not using or changing PPE. Drivers of staff using garden as a toilet.
