Sharon has hands on experience in: Nottingham home care from a previous employer in helping people in need of support to set up their own aims and type of support and care they needed for their health and holistic wellbeing. Sharon had to gather information to come up with a detailed document setting out t extra help needed to manage their lives and be as independent as can be in the clients 'condition s very experienced in producing a detailed document setting out what services required, how those needs will be meet through specific service provision by specific care supporters. As a result, Sharon has acquired a lot of experience in using the key to unlock person-centred co-ordinated care which ensures consistency of same quality of care whilst equally reviewing the needs according to the changes if any in the clients' situations. Sharon’s experience as Team leader, Care coordinator, and Support worker as well as a Support Planner and Support worker is a wealth of experience.
