The Referral Manager at National Medicare Services is responsible for overseeing the referral process and ensuring individuals are matched with the most appropriate care and support services. Working closely with local authorities, NHS teams, hospitals, Integrated Care Boards (ICBs), case managers and private clients, the Referral Manager coordinates referrals from initial enquiry through to service commencement.
The role involves assessing referral information, identifying suitable placements or care packages, liaising with healthcare professionals and families, and ensuring a smooth and timely transition into care. The Referral Manager works collaboratively with operational teams to ensure services can safely meet each individual's needs while maintaining high standards of person-centred care.
In addition to managing referrals, the Referral Manager develops and maintains professional relationships with commissioners, discharge teams, social workers, community mental health teams and other health and social care partners. By acting as a central point of contact, the role helps facilitate effective communication, promotes continuity of care and supports positive outcomes for the people referred to National Medicare Services.
