Your role
You will be at the forefront of delivering high quality,
compassionate, person-centred care to all of our service users.
You will build strong and trusting relationships with them, their
families and loved ones, and the wider local community.
You will enable all of our service users to remain as independent as
possible whilst maintaining their dignity and freedom of choice.
You will be based in our service user’s homes within the community.
Your responsibilities
Performance and growth
- Attend visits allocated to you through our digital care management system.
- Deliver our service user's care and support plans. Provide
personal care (for example washing) and emotional and practical
support. Recognise when there are updates and changes in service
user needs and ensure proposals are reported to senior staff for review.
- Carry out domestic tasks such as cleaning, laundry and shopping.
- Assist with the preparation and provision of hot meals to our
service users.
- Regularly check our service user’s well being as directed, and
according to the needs detailed in their care and support, and risk, plans.
- Deliver high quality, compassionate, person-centred care to our
service users living with dementia. Suitable training will be provided.
- Use systems to ensure smooth delivery of service, including our
digital care management software.
- Develop an awareness of other local community services and build
effective and professional working relationships with them,
including other health and social services.
- Sign-post our service users and their loved ones to local
community services, where relevant.
- Remain proactive and provide feedback to the management team about
your own training and personal development needs.
- Contribute to effective team working and collaboration, so that
you are part of a positive workplace culture.
- Support your colleagues by ensuring visit reporting and handovers
are clear.
- Attend meetings and other activities as directed by your line
manager. They may be outside of your regular shift pattern.
- Undertake any other tasks relevant to your role, as directed by
the Registered Manager.
Regulatory
- Deliver all services in accordance with the Care Act, Care
Standards Act, and other relevant regulatory frameworks.
- Work with colleagues from statutory and voluntary organisations to
provide joined up care.
- Advocate on behalf of service users with other agencies.
- Appropriately respond to emergencies. Contact emergency services,
deal with the breakdown of services e.g. power cuts, respond to
death, and keep detailed and accurate records in all cases.
- Work within relevant health and safety standards. Raise any risks
or issues in a timely manner with senior staff, using out of hours
on call where necessary.
- Ensure that our administrative policies and procedures are
followed, and that records from our service users remain up to date.
- Keep your professional knowledge and skills up to date, including
on relevant legislation and mandatory internal training.

Your skills / experience
Essential
Education
- GCSE / O-Level education, including English and Maths
- NVQ Level 2 in Health and Social Care or equivalent
Skills
- Work a range of shifts to meet service user needs
- Build trusting and constructive working relationships with
colleagues, service users and their loved ones, and members of the
wider local community
- Excellent verbal and written communication skills
- Knowledge on how to deliver integrated care with other health and
social care services
- Work independently and act on your own initiative where appropriate
- Right to work in the UK
- Valid UK drivers licence, access to a car, and suitable and valid
business car insurance

Desirable
Experience
- Experience caring for older people
- Experience using IT or other digital systems to write and report
care notes, access rotas and schedules, and complete care visits
- Experience working in partnership with carers, volunteers, the
loved ones of service users, and members of the community to achieve
high quality outcomes for service users
- Experience working with service users who have complex diagnoses,
such as those with learning disabilities, dementia, and Alzheimer’s
- Experience working night shifts, including waking nights
Benefits package
- Basic pay of £13.00 per hour
- Paid travel time between service user visits at your basic pay rate
- Paid mileage between service user visits at £0.45 per mile
