Job summary

Job Title: Care Coordinator
Salary: £22,000 to £25,000 depending on experience
Working Hours: Monday to Friday - 10 am to 6 pm with possibility of on-call duties at the evening and weekend

Purpose of Role:
The Care Coordinator will play a crucial role in ensuring our Service Users receive top-quality care and support to maintain their safety and comfort at home. This position requires a supportive and calm approach, coupled with excellent organizational and communication skills. 

Key responsibilities: 

  • Collaborate with service users, their care circle, and healthcare professionals to ensure a clear understanding and delivery of quality care.
  • Assist the Care Team in planning effective support worker rotas, matching worker skills to service user needs.
  • Conduct initial assessments and regular reviews of care packages, updating needs and risk assessments as necessary.
  • Provide support and guidance to support workers, ensuring their understanding of service users' needs and promoting safe working environments.
  • Record and escalate concerns, complaints, and compliments appropriately, and effectively share information with relevant professionals.
  • Run weekly audits 
  • Support with "On Call" 

Skills / Qualifications:

  • The ideal Care Coordinator candidate will hold at least an NVQ Level 3 in Health and Social Care with previous experience within a domiciliary care agency/residential home for at least one year. We will require the successful candidate to be able to start work from day one and hit the ground running.
