Job summary
Location: Stockton-on-Tees
Pay: £12.50 per hour + 35p mileage
Shifts: Mixture of days, nights, two evenings per
week & alternative weekends
Work Location: On the road (community & supported housing)

This is a community-based role. Applicants must
hold a full UK driving licence and have
access to their own vehicle. We are unable
to offer sponsorship for this role.
Key Responsibilities
As a Community Support Worker, you’ll provide person-centred support to individuals in their own homes. Your daily responsibilities may include:

Assist with daily living tasks, including personal care, cooking, cleaning, shopping, and budgeting

Provide emotional and social support, companionship, and encouragement to engage in community activities

Support with medication, basic health monitoring, and accompany individuals to appointments

Help individuals develop life skills, pursue education, or gain employment

Maintain accurate records and care documentation
-
Work closely with families and healthcare professionals to deliver consistent, person-centred care

Follow personalised support plans tailored to individual needs and goals

A full UK driving licence and access to your own car (essential)
