Clinical Care Manager (Healthcare Professional)

We are now looking for a Clinical Care Manager to join our growing team — a role ideal for a healthcare professional who wants to use their clinical background to shape high-quality care in the community.
This is not a CQC Registered Manager role. You will report into the Registered Manager and focus on clinical oversight, quality, and supporting both clients and care teams.

Your Role:
As a Clinical Care Manager, you will play a key role in ensuring safe, effective, and compassionate care is delivered in people’s own homes. You will use your professional background to guide quality, support care staff, and work collaboratively with families and healthcare partners.
Your responsibilities will include:
- Providing clinical and professional oversight to care packages and care teams
- Supporting the Registered Manager with quality assurance, audits, and service development
- Managing a caseload of clients, ensuring care remains person-centred and outcomes-focused
- Acting as a key point of contact for families, advocates, and external health professionals
- Supporting care planning, risk assessments, and reviews
- Contributing to staff training, development, and reflective practice
- Offering support in the field only as a last resort, ensuring continuity and safety of care
What We’re Looking For:
- A background as a healthcare professional — ideally a Registered Nurse, but we also welcome applications from physiotherapists or similar regulated professionals
- A strong understanding of clinical risk, safeguarding, and holistic care
- A compassionate, values-driven approach to leadership
- Confidence working in community-based care and supporting people to remain at home
- Organised, proactive, and an excellent communicator
- Experience in care management, clinical leadership, or a role ready to step into this level of responsibility
