Job summary
Job Title: Support Worker / Care Assistant
Reports to: Registered Manager/care coordinator
Overall Purpose:
To deliver high-quality, person-centered care and support to individuals in their own homes or supported living settings. The role promotes independence, dignity, choice, and well-being in line with Premier Haven Ltd’s values and the principles of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
Key Responsibilities:
- Provide personal care and practical support to service users in a kind, respectful, and professional manner, maintaining dignity and independence at all times.
- Support with daily living activities, including personal hygiene, dressing, meal preparation, medication, and mobility.
- Follow individual care and support plans and contribute to ongoing person-centred planning.
- Administer medication safely (using the approved eMAR system, where applicable) after completing all required competency assessments.
- Assist with household tasks such as cleaning, laundry, and shopping, as required.
- Support service users to participate in social, educational, and recreational activities, both within the home and the community.
- Observe and report any changes in the service user’s condition or circumstances promptly to the Duty Manager.
- Maintain accurate and timely records using Person Centred Software (PCS) or equivalent systems.
- Ensure infection control procedures and health and safety requirements are followed at all times.
- Treat all personal and sensitive information confidentially and in accordance with data protection policies.
- Attend training sessions, supervisions, and staff meetings as required.
- Mentor and support new team members when requested.

Day-to-Day Duties:
- Provide compassionate and reliable care in line with care plans and agreed outcomes.
- Ensure safe use of equipment and assist with moving and handling, following training and risk assessments.
- Encourage service users to make informed choices and participate in decisions about their care.
- Record care delivery accurately and report concerns immediately.
- Support the maintenance of a safe, clean, and comfortable environment.
- Respond promptly to service user requests and communicate effectively with families, professionals, and colleagues.
Skills / Qualifications:
- Qualifications & Experience: • Previous experience in care (preferred but not essential). • Completion of
