Home Care worker

To provide health care support for persons with health and social care needs within all environments primarily in their own home.
To endeavour to meet all required "Outcomes" and re-enable/enable, encourage, support, or assist the service user with their activities of daily living and their needs as identified in the "Service Delivery Care Plan".

To maintain and improve their quality of life and independence.
To assist management to capture and record "Outcomes Compliance" to evidence Quality Assurance.
