Job Summary
To provide safe, person-centred care and practical support to individuals in their own homes or supported living environments. The Care Worker is responsible for delivering care in line with agreed support plans, promoting independence, safeguarding wellbeing, and maintaining accurate records. The role is primarily field-based, with associated administrative duties completed digitally or during team meetings.
Key Responsibilities
- Personal Care Delivery
- Support individuals with washing, bathing, dressing and grooming.
- Assist with toileting and continence care in a dignified manner.
- Support mobility, including safe transfers using appropriate equipment.
- Assist with eating and drinking where required.
- Encourage independence wherever safely possible.
- Medication Support
- Prompt or administer medication in line with training and company policy.
- Complete Medication Administration Records (MAR) accurately.
- Report medication errors, refusals or concerns immediately.
- Daily Living and Practical Support
- Prepare meals in line with dietary needs and preferences.
- Support with shopping, light domestic tasks and laundry where included in the care plan.
- Accompany individuals to appointments or community activities if required.
- Provide companionship and emotional reassurance.
- Monitoring and Reporting
- Observe and document changes in health, behaviour or wellbeing.
- Escalate safeguarding concerns without delay.
- Report accidents, incidents and environmental risks promptly.
- Maintain clear, factual and contemporaneous electronic care records.
- Compliance and Professional Standards
- Follow care plans and risk assessments precisely.
- Work in line with CQC regulations, safeguarding legislation and company policies.
- Adhere to infection prevention and control procedures.
- Maintain confidentiality and comply with data protection requirements.
- Attend mandatory training and supervision sessions.
- Communication
- Communicate professionally with service users, families and colleagues.
- Liaise with senior staff regarding changes in care needs.
- Report rota issues or service delivery concerns promptly.
