About the Role
You will support the success of our Chichester branch by coordinating safe, effective and compassionate care delivery while helping the branch meet its quality, compliance and growth objectives.
This role is ideal for an experienced care coordinator or planner who enjoys working in a busy, people-focused environment and is confident managing rotas, staffing requirements and changing client needs.
Main Responsibilities
- Ensure all scheduled client visits are covered each day.
- Prepare and maintain rotas at least four weeks in advance.
- Allocate care staff according to client needs, preferences, skills and continuity requirements.
- Coordinate quality visits for Field Care Supervisors and Field Care Managers.
- Liaise with Branch Nurses regarding complex care assessments, training and competency checks.
- Keep care teams informed of changes to client needs and care plans.
- Maintain strong relationships with clients, families, frontline colleagues and external professionals.
- Respond promptly to staffing issues, client concerns and changes in care requirements.
- Work closely with healthcare professionals, social workers, case managers and multidisciplinary teams.
- Ensure records, office systems and procedures are accurate and compliant.
- Prepare reports as requested by management.
- Support local business development opportunities alongside the Branch Manager.
- Participate in the on-call rota, including approximately one weekend in four.
- Support neighbouring branches when required.
- Contribute to the day-to-day running of the branch.
- Carry out other reasonable duties as directed by the manager.
Essential Requirements
- Previous experience in a care coordination, scheduling or planning role.
- Strong organisational and time-management skills.
- Excellent communication and interpersonal skills.
- A proactive and solution-focused approach.
- Ability to work calmly in a fast-paced environment.
- Confidence using scheduling, rota or care management systems.
- Ability to manage competing priorities and respond to urgent situations.
- Commitment to equality, diversity, inclusion and person-centred care.
- Willingness to participate in the on-call rota.
Desirable
- Full UK driving licence and access to a vehicle.
- Experience in complex or nurse-led care.
- Knowledge of home care quality and compliance requirements.
- Confidence promoting branch services to potential clients and professional contacts.
- A strong interest in continuous improvement and service excellence.
