Job Summary

To provide high-quality, person-centred care and support to clients in their own homes or community settings.

Promote independence, dignity, and safety while assisting with daily living tasks.
- Ensure adherence to care plans and organisational standards while maintaining compassionate and reliable service
- Assisting with personal care, including bathing, dressing, grooming, toileting, and mobility support

Meal preparation and nutrition support, including feeding if required.

Supporting with medication according to care plans and organisational procedures.

Performing domestic tasks such as light cleaning, laundry, changing bed linens, and shopping.
- Providing companionship and emotional support to reduce social isolation

Accompanying clients to appointments, social activities, or community events.

Monitoring and reporting changes in client wellbeing to supervisors or care managers.
