Job Summary

Prompt Healthcare Staffing is looking to recruit an experienced Care Coordinator Manager to join our team based in Basildon Essex. The Coordinator will be performing a variety of duties in this role of coordinating a service for clients. This is a management, hands-on role assisting and supporting other staff members to provide the highest quality service to our Service Users.

This care service involves a program of personal care and household care management that is personalized for each Service User in the form of a care plan. Care Coordinator duties will therefore include Liaising with Outside Contract Managers, Brokerage Team, carrying out assessments, Organising reviews, carrying out Quality Assurance, Spot Checks, Supervision and Care Review. 

Principal responsibilities – Care Coordinator

Responsibilities:

· Contact and visit service users as directed by the organization to carry out service user Assessments, Reviews and Meetings.

· Ensure that All New Service Users are contacted and Assessments booked.

· Work closely with Office Team- to ensure the smooth running of the Organisation

· On acceptance of a New Service User, liaise with the Service User, family and other professionals accordingly.

.  Liaise with Placement team, family, and other professionals throughout the duration of a package of care.

· Ensure that Carers and Service Users Rota are robust to ensure a smooth running of the Organisation to meet the needs of Service Users.

· Ensure all paperwork in the service users’ profile has been completed and service user information recorded in line with company policy. Check that all forms are being completed correctly. Remove and replace used forms as required.

· Report on service user visits, noting any concerns regarding service users or care/ support workers, to management.

· Participate in the On Call Rota for the smooth running of out of hours Service as needed.

· Visit new service users. Ensure that the service user is aware of policies and procedures. Obtain signatures on contracts and other documentation as required by management.

· To participate in, and generally support, the supervision of care/support workers as required. To join or lead regular team meetings (peer group supervision) or individual meetings (one- to-one and Group supervision) to review any individual care concerns, discuss service users’ requirements and update care/ support workers on administrative and policy matters. To record and return minutes of these meetings and to discuss the outcome of such meetings with management as and when required.

· To participate in Work-shadowing and the integration of care/ support workers into their new working environment.

· To liaise with management in supervising office staff.

· To cover Care Visits if required

Schedule:

  • 0900-1700
  • Monday to Friday
  • On call Out of hours 1week per month

Ability to commute/relocate:

  • Have own car.

Education:

  • A-Level or equivalent 
  • NVQ level 3 or equivalent 

Experience:

  • Providing care: 3 years (required)
  • Domiciliary Care Coordinating: 1 year (required)
