Job Summary

We are seeking compassionate and dedicated Home Care Support Workers to join our professional team. The ideal candidate will play a vital role in providing high-quality, person-centred care to individuals within their own homes.

You will support clients in the comfort and familiarity of their own homes, helping them maintain their independence, dignity, daily routines, and quality of life. This may include assisting with personal care, household routines, mobility, appointments, and other activities of daily living in accordance with individual care plans.

The role requires strong communication skills, reliability, compassion, and the ability to work collaboratively with clients, their families, and healthcare professionals.

Responsibilities

  • Provide compassionate, person-centred care and support to clients in their own homes.
  • Assist clients with personal care tasks, including bathing, dressing, grooming, and other activities of daily living.
  • Support clients with daily routines and activities as outlined in their individual care plans.
  • Encourage clients to maintain their independence, dignity, choice, and quality of life while living at home.
  • Provide practical support within the client's home, where required, to help them manage their everyday routines safely.
  • Communicate effectively with clients, their families, and healthcare professionals to ensure continuity and consistency of care.
  • Drive or accompany clients to appointments, social activities, or other community activities as required.
  • Monitor and report any changes in clients' health, wellbeing, behaviour, or circumstances to the appropriate personnel.
  • Maintain accurate and timely records of care provided and contribute to the development and review of individual care plans.
  • Follow all relevant care plans, risk assessments, safeguarding procedures, and organisational policies.
  • Help maintain a safe, clean, and comfortable environment for clients within their homes.
  • Respect clients' privacy, confidentiality, cultural preferences, personal choices, and individual needs at all times.

Essential Criteria

  • Previous experience in home care, domiciliary care, supported living, or a similar care role.
  • Understanding of care plans and the ability to follow them accurately.
  • Experience or understanding of providing care and support within clients' own homes.
  • Excellent communication skills in English, both verbal and written.
  • Basic IT skills for documentation and record-keeping purposes.
  • A compassionate nature with a genuine desire to help others improve their quality of life.
  • Ability to work independently while also working effectively as part of a wider care team.
  • Reliable, responsible, and committed to providing high-quality care.
