Specialising in supporting individuals to remain in their own home and promote their independence. We pride ourselves on ensuring each client is provided with care visits to meet their needs and requirements. Due to the growth of our operations, we now require additional office support. A position is available to join our successful team as a Care Coordinator.

Duties

  • Act as the primary point of contact for clients, families, and healthcare providers, ensuring clear and empathetic communication at all times
  • Develop and implement personalised care plans tailored to each client’s unique needs and circumstances
  • Coordinate appointments, services, and resources efficiently using IT systems to streamline rota scheduling and documentation
  • Educate clients and their families about available services, health management strategies, and community resources through engaging teaching methods
  • Monitor client progress regularly, adjusting care plans as needed to optimise outcomes and conduct monthly medication and care visit audits
  • Maintain accurate records of interactions, service delivery, and progress notes in compliance with privacy regulations
  • Collaborate with multidisciplinary teams to ensure comprehensive support that aligns with each client’s goals

What we look for

  • Excellent communication skills with the ability to listen actively and convey information clearly in English
  • Strong IT proficiency for managing electronic health records, scheduling software, and other digital tools
  • Experience in the social care sector (home care preferable), organised with proactive initiative and a competent approach
  • Ability to work independently while maintaining a collaborative approach within a team environment
  • Compassionate demeanour with a genuine desire to support individuals through their care journey
  • Prior experience in healthcare or social services is preferred but not required; training will be provided for motivated candidates

Experience:

  • Adult Social Care or Home Care: 1 year (required)
  • Care Coordinator: 1 year (preferred)
