Overview
Care Relationship Manager (CRM)
Operates directly alongside the Service Manager. CRM focuses entirely on the client journey, relationship building, quality assurance, and compliance.
To serve as the primary, professional face for all/selection of service users, families, and external professionals. The CRM is responsible for managing the end-to-end client relationship, including conducting initial home assessments, drafting compliant person-centred care plans, managing complex family communications, and leading review processes to ensure care delivery consistently exceeds expectations.
Key responsibilities -
- Initial Assessments: Conduct thorough, dignified initial assessments in the homes of prospective clients.
- Person-Centred Care Planning: Design, draft, and implement comprehensive Care Plans and Risk Assessments in strict alignment with Regulation 15 of the Regulated Services (Wales) Regulations 2017
- Regulatory Alignment: Audit incoming local authority files to ensure that official Social Services Care Plans and commissioning agreements match our internal tasks before care begins.
- Risk Detection: Identify and log risks during home assessments and coordinate urgent referrals to external professionals like Speech and Language Therapists (SALT) or Occupational Therapists (OT).
- Family Point of Contact: Act as the primary relationship manager for service users and their families. Address feedback, mitigate worries and manage communication
- Partnerships: Build and maintain excellent professional relationships with Local Authority Social Workers and healthcare teams. Co-ordinate and lead multi-agency joint care reviews to align commissioned care goals with real-world delivery.
- Conflict Resolution: Act as the first point of contact to investigate and de-escalate service delivery complaints, ensuring families receive empathetic, swift, and formal written solutions.
- Field Quality Spot Checks: As well as seniors/trainer conduct spot checks on live care calls. System Audits: Audit active client files, medication records (E-MAR charts), and communication logs.
- CIW Inspection Readiness: Maintain detailed documentation of all client reviews, complaints, and risk amendments to provide an audit trail during inspections.
