One of Transitions' aims is to match carer with the client as far as possible and they achieved this splendidly. They also provide the same carer for five of the seven visits which matter a great deal. help with washing, shaving, showering and dressing is given sensitively, along with encouragement towards continued independence. Time for conversation afterwards is generously given, which is particularly valuable an appreciated as a help towards restoring sociability. Kindness, concern and respect feature largely in Transitions car of clients and offers and provision of help make life seem more secure.
When I first got with Transitions I was very sceptical about the service they provided because it wasn't an ordinary care company. I had very low self-esteem and being with Transitions they have included me in team meetings. Gone to college to get some certificates. They have helped me change my house quite a lot. It was very bare when Ic same into my house but with some decor, we have changed it from a house to my home. I cannot recommend Transitions more so.
Transitions Care ensure my care needs are completely met with no fuss from all participants. Small changes in daily care routines are arranged freely without problems e.g. whether to have a bath or not, whether to have my hair washed or not. Far more care facilities are available than I wish or need to take advantage of from Transitions Care.
We contacted Transitions Care in March 2020 when looking for some home support for my elderly in-laws who live quite a distance from us. We were very impressed with the director and felt immediately at ease due to his clear understanding and caring nature. The support required was discussed at length with both my in-laws and ourselves and began immediately. A very small number of carers work with my in-laws and have all been introduced prior to beginning the care/support. Having said that, 90% of the care has been provided by one carer and the difference she has made has been incredible. She has provided security and assistance, encouraged independence and been an absolute lifeline (continuing daily visits) during the COVID-19 pandemic. We cannot praise Transitions enough. Highly, highly recommended.
I am deeply indebted to Transitions. I had to arrange care for my mother and partner from a long way away, without the possibility of meeting them in person first. The manager put my mind at rest over the phone instantly, and the care package we have had in place since last summer has been very professional and consistently reviewed. The team have always gone the extra mile and they are proactive in the approach to their care. I have since met them all and they are of course all very lovely. Thank you to the team.

Fantastic company, best decision we made as a family for my mother who has recently been diagnosed with Alzheimer’s. A certain member of staff goes above and beyond her role. Made a fantastic relationship with my mum and family members. They really are worth their weight in gold.
Recently, my nan was diagnosed with Alzheimer’s. We chose as a family Transitions Care Fylde and it was the best decision we could have made. One staff member is an asset to the team and as a family, even over a short period of time, we have noticed that she is very professional and so kind and caring with my nan. Her communication with the whole family is fantastic and proves that the support is not just for my nan but my family too. She has a huge heart and loves every part of her job which stands out from miles. We will be forever grateful and appreciative of her and all the love and support we receive! The care package was agreed between my mother, the family and Transitions which met her care needs, supporting visits for shopping, to hairdressers and trips for coffee. As her needs increased the care package was increased to ensure mum continued to be appropriately supported.
Transitions staff are well trained and suitably qualified. They have the client's needs at the heart of their service.
The team of carers are well managed. There was a consistent team visiting my mother. This consistency of staff was particularly important to my mother as she developed good relationships with each of them.
Transitions showed great flexibility in responding to unexpected events.
I cannot praise Transitions highly enough, they are a first-class care provider.
There is nothing negative to say about the care Transitions Care provide to/for my Mum. We engaged them through the direct payment option some 2 years ago and it has been a great decision. I hope they get an "Outstanding" score in their recent CQC review - they deserve it.
My mum had care from Transitions for only a few weeks before she was hospitalised but those few weeks made her very happy. As a nurse, I cannot fault the care she received and I am very fussy! I could relax knowing she was in good hands. The staff were friendly and obviously happy in their work and they treated my mum with the utmost respect. Highly recommended.
Having a small, regular team visiting, on time and not in a rush to do the job and hurry away has worked well for Mum and is what she has asked for.
Mum can feel lonely in what to her is a long day, but the carers always take time to chat to Mum, have a laugh and take the wrappings off the chocolates for her!
It's a comfort to know that Transitions carers care for my mum.
The Manager of Transitions and his team of dedicated Carers have been looking after our Mum for the last four months and we are extremely pleased with the service provided. From the initial meeting at home with Mum, to find out her personal needs and requirements, the Manager has overseen many aspects of Mum's personal care. Mum wanted as few Carers as possible and at regular times. This has all been provided - a team of four lovely Carers who arrive at the regular times Mum wanted. We have seen a big improvement in Mum's level of anxiety, along with the laughs and hugs she receives from her Carers that make her day feel less lonesome. Knowing Mum is looked after so well is very reassuring and a credit to the Transitions ethos.

David is currently studying for his level 3 Diploma in Health and Social Care.
Debbie joined the Transitions team in 2023 with vast experience, especially in dementia care. Debbie is currently being supported to progress in her role as a care coordinator and is a highly valued member of the team.
