Patch Care Home Care - A flexible new care service that is designed to combat loneliness where care staff are allocated to an area to care for a certain number of clients within the area to become their named key worker to encourage good relationship and confidence.

Permanent allocation of care staff - They allocate carers according to their skills, experience and culture to clients to encourage continuous care.

Escort - They are able to escort clients to their appointments, social activities and visit families and friends.

Live in care - They assign carers to live in with clients in their own house to manage their day to day living activities as well as being their friend and fit in to be part of their family.

Complex care - For clients who have complex health needs in the home, for example, PEG feeding.

Domestic help - For clients that are unable to maintain a healthy environment.

Cultural and belief sensitivity - They provide staff that speak community languages to promote individualised, better care and good relationship. Carers are trained to respect individual cultural beliefs and support clients' individual care needs.

Befriending service - To enhance social and emotional well-being, both on the phone and in the house.

General personal care and support - Which includes prompting and supporting with medication and meal preparation to meet daily individualised needs.

Relieving family - That look after their loved ones in their home to offer respite.

Fantastic care for both parents from both dedicated carers. The ability to speak the native tongue and have shared life experiences makes a huge difference. As well as medicine and assistance in the home, they are integral to long term well being by increasingly expanding the physical capabilities
of my mother following her recent hospital stay. This has built on and extended the NHS support. Being medically trained in their original countries has been significantly valuable to this. They both always have a smile for everyone; but equally have my parents care right at the centre of their roles. They both like to be busy and keep my parents occupied. Yet they are sensitive to the needs of my parents and flex and integrate with that. Such a brilliant overall experience. We would be completely lost without them.

My Mother's carer, is 100%. She goes above and beyond for her and can't do enough for my Mother. She even phone's my Mum in her own time to make sure she's taken her medication. Can't recommend them more.

I wanted to give an update on the fantastic Carers that look after my mum these girls are great have been with my mum since she came out of hospital on end of life care back in 2023 and mum is now back with us still yes she may be bed bound now but she has been looked after so so well and mum is happy
I still say if it wasn't for two members of staff in particular that she has had from the start we would have lost her she loves them so much .they are now part of our family and is so nice to have the same girls that got to know her needs so much some times we have other girls to help out if two staff members in particular can't make it and so far these girls have been great so careful with mum one staff member in particular trains them well .so thank you so so much we wouldn't be with out them life for mum is so so much easier when she trusts theses girls because at 92 and mum is fully aware what's happening that these girls are gentle and kind .you have great girls working for you they are Angels xxxx THANK YOU So MUCH

One of the carers massaged my friend’s legs which reduced the swelling of the legs. None of the other carers thought of it. I’m glad that particular carer did it so my friend is at ease afterwards. Hopefully, they will look after my friend well.
