We are seeking an experienced, motivated and highly organised Care Coordinator to join our team and take an important role in the coordination and delivery of our services across the Sutton branch.

The Role
The Care Coordinator will be responsible for organising, coordinating, monitoring and continuously improving the delivery of high-quality care to our service users.
You will act as an important link between service users, their families, care staff, management and external professionals, ensuring that care is delivered safely, consistently and in accordance with individual needs.
The successful candidate will be expected to demonstrate strong leadership, excellent communication skills and a thorough understanding of care standards and regulatory requirements.
Key Responsibilities
Care Delivery & Coordination
- Develop, review and update personalised care plans in line with assessments and changing service user needs.
- Conduct initial assessments, care reviews, spot checks and welfare visits.
- Coordinate and allocate care staff appropriately, taking into consideration skills, availability, location and service user requirements.
- Ensure continuity and punctuality of care delivery.
- Monitor service user wellbeing and promptly escalate concerns where necessary.
- Maintain positive and professional relationships with service users and their families.
- Respond effectively to changes in care requirements.
- Monitor the quality and consistency of care being delivered.
Rota & Staff Coordination
- Prepare and manage staff rotas effectively.
- Ensure appropriate staffing levels and suitable care worker allocation.
- Manage changes to the rota and respond to short-notice staffing issues.
- Coordinate emergency cover where required.
- Monitor staff attendance, punctuality and reliability.
- Promote continuity of care wherever possible.
Compliance & Quality Assurance
The Care Coordinator will play a key role in ensuring the service remains compliant with relevant legislation, regulatory requirements and organisational policies.
Responsibilities include:
- Ensuring care delivery is consistent with CQC requirements and regulatory standards.
- Working within the principles of the Care Act 2014 and Mental Capacity Act 2005.
- Promoting effective safeguarding practices.
- Maintaining accurate and up-to-date care records.
- Completing and maintaining risk assessments, incident reports, care notes and other required documentation.
- Supporting audits, quality monitoring and compliance checks.
- Conducting care quality spot checks.
- Identifying areas for improvement and supporting continuous improvement initiatives.
- Promoting safe medication practices.
- Supporting effective infection prevention and control.
- Ensuring health and safety procedures are followed.
- Escalating incidents, safeguarding concerns and compliance issues appropriately.
Team Support & Leadership
- Assist with the recruitment and onboarding of care staff.
- Support staff induction and ongoing development.
- Provide guidance, coaching and day-to-day support to care workers.
- Promote professional standards and good practice across the care team.
- Support staff supervision and performance management where required.
- Encourage effective communication and teamwork.
- Ensure care workers understand and follow care plans, risk assessments and company procedures.
Communication & Professional Liaison
The Care Coordinator will be expected to build effective professional relationships with a range of individuals and organisations, including:
- Service users and their families.
- Care workers.
- Social workers.
- Nurses and GPs.
- Occupational therapists and other healthcare professionals.
- Local authorities and commissioning teams.
- Other relevant multidisciplinary professionals.
You will:
- Respond professionally and promptly to enquiries, concerns and complaints.
- Represent Premium Homecare Ltd at meetings, reviews and professional discussions.
- Communicate changes in service user needs effectively.
- Ensure important information is shared appropriately with relevant members of the care team.
- Maintain confidentiality and professional boundaries at all times.
Essential Requirements
Qualifications
- NVQ/QCF Level 3 in Health & Social Care – minimum requirement.
- Level 4 or Level 5 qualification in Health & Social Care is desirable.
Experience
The successful candidate should have:
- Previous experience within domiciliary care, supported living or care coordination.
- Experience of coordinating care staff and service user schedules.
- A strong understanding of safeguarding and care quality requirements.
- Experience of care planning, risk assessment and incident management.
- An understanding of CQC regulatory expectations.
Skills & Competencies

Additional Requirements
The successful candidate will be required to:
- Complete an Enhanced DBS check.
- Hold a full UK driving licence and have access to a vehicle where required for community-based duties.

- Be able to work effectively within a busy domiciliary care environment.
- Maintain confidentiality and comply with data protection requirements.
Desirable Experience

- Experience supporting individuals with complex care needs.
- Experience supporting people living with dementia.
- Experience supporting people with learning disabilities.
- Experience within mental health services.
- Medication training or experience as a medication trainer.
- Moving and handling training or trainer experience.
- Knowledge of local authority and commissioning processes.
- Experience working with local authority systems and multidisciplinary teams.
