Domiciliary Care Manager

We are seeking a compassionate and experienced Care Manager to join our team. As a Registered Manager, you will play a crucial role in ensuring the well-being of our service users. You’ll work closely with our care team, service users, and their families to provide personalized support.

Your role will involve managing care plans, coordinating with healthcare professionals, and ensuring the well-being of our Service users.

Duties:
- Develop and implement individualised care plans for each Service User based on their specific needs and preferences
- Conduct regular assessments to evaluate the effectiveness of care plans and make necessary adjustments
- Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive care for Service Users
- Provide leadership and guidance to direct care staff in delivering high-quality care
- Maintain accurate documentation of Service Users assessments, care plans, and risk assessments.
