Crossroads offers personalised care and support packages to help you to stay in your own home for as long as possible.

Care is person-centred and can be provided to people of all ages, disabilities and health conditions, 24 hours daily, with access to emergency support.

If you live in Sandwell or the surrounding areas, the team of Care Support Workers can provide tailor-made, flexible support to meet your individual needs. This can include:

- Personal care & rehabilitation support
- Meal preparation
- Help with shopping and chores in the home
- Support with taking medication
- Companionship
- Help to get out and about to socialise or to attend hospital or GP visits
- Respite breaks
- Palliative care
- Night sits

Their services can be accessed in a variety of ways. They are able to advise you if you are self-funding or eligible for statutory funding to cover care costs.

Crossroads also offers a Day Club which strives to support the health and wellbeing of people who require care as well as their family carers. A skilled team of Crossroads Care Support Workers are on hand at the Day Club to assist people with mealtimes and toileting needs if required.

The Day Club is held on the first Wednesday of each month from 10am - 3pm at The Oak Tree Centre.
