We deeply apologise for the distressing experience and failure to recognise and respond to multiple seizures. We acknowledge the profound distress caused by our shortcomings in care quality and internal communication The safety and wellbeing of those we support are our top priority, and we are truly sorry we fell short of our high standards. A comprehensive internal review has been completed, assessing staff training, competency, and adherence to protocols for managing medical emergencies, particularly seizure recognition and response. We have also reviewed our internal communication systems and supervision process to ensure staff are adequately supported and critical information is shared effectively. We are committed to learning from this incident and taking decisive action to reinforce our commitment to providing compassionate, competent, and safe care We would appreciate the opportunity to discuss this matter directly with you again to update you on the specific actions we have taken.
