Jacob Joseph brings 10 plus years of NHS experience to his role as Clinical Care Coordinator at Axiom Home Care. Throughout his healthcare career, he has worked across a wide range of clinical settings, including acute stroke units, elderly care wards, intensive care units (ITU), rapid response services, integrated case management, and early supported discharge teams. He has also supported nursing homes across the Brent area as an Advanced Care Practitioner.

Working closely alongside the Registered Manager and Deputy Manager, he supports the delivery of high-quality, person-centred care across the organisation.
