At Vitale Care, it’s a member of our Care Delivery Team who comes to meet you in your home. During the visit, they’ll discuss your usual routines, any difficulties you may be experiencing, and the type of assistance that could make everyday life easier. They’ll also discuss your personal preferences and whether companionship is required. If you’d like a family member or friend present, that’s completely welcome; in fact, we encourage it.
Part of the visit may involve looking around the home environment. This is not about judging your home in any way; it simply helps ensure care can be provided safely and comfortably.
Most importantly, the assessment gives you the chance to ask questions and share any worries or expectations you may have.
Understanding the Person Behind the Care
Quality home care is about much more than practical tasks like meal preparation or getting dressed. Truly good care comes from understanding the individual, their personality, lifestyle, interests, and the things that bring them happiness and comfort.
That’s why we spend time during the assessment getting to know what matters most to you. We’ll talk about your routines and preferences, whether you enjoy social activities, or if there are certain times of day when you appreciate company or quiet time. We may discuss favourite meals, hobbies, television programmes, or anything else that helps us understand how to make support feel natural and personal.
These conversations allow us to create care that fits around your life and routines, rather than expecting you to adapt to the care itself. Our aim is always to help people continue enjoying life in the comfort and familiarity of their own home.
Creating the Right Support Plan
Following the assessment, the information gathered is used to create a personalised care plan tailored to your needs and preferences. It also helps our Care Delivery Team choose Care Professionals who are not only suited to the practical side of support, but who are also a good personality match.
Care plans are not fixed permanently. They are reviewed regularly, both after the initial settling-in period and whenever circumstances change. As needs evolve, care should evolve too.
